Hispanic Heritage Month Nomination Form

Union Bank and KCET, public media for Southern and Central California, are looking for outstanding Latino men and women who are providing great leadership and making significant contributions to improve and enrich the lives of others. The stories of selected heroes will be broadcast on KCET during Hispanic Heritage Month (September 15 - October 15).

To submit a nomination, review the guidelines, complete and submit the form below or email or mail the pdf form. The deadline to submit nominations is Friday August 2, 2013.

Substance abuse facilities typically focus on helping individuals get better in a safe environment but overlook the aesthetics of the individual's surroundings. Beit T'Shuvah in L.A. has taken a big step further, thanks to 70 designers.
